A COVID-19 pre-screening questionnaire and temperature check 24 to 48 hours in advance of your appointment, and upon arrival 10 minutes prior to your appointment, are required. You can download the pre-screening form in English here or in French here and email it to your doctor’s receptionist (linked below). Phone pre-screening is also available for those without email by calling 613-824-3888 and going to the proper extension:
